Completing New Patient Health History Forms

If you were not able to complete the new patient health forms online, please arrive about 15 minutes early for your appointment to complete these forms for the doctor's use during exam + consultation.

DAY 1

Exam and Consultation

During your first visit, Dr. Shelley will go over your health history forms, complete X-Rays if needed as well as spend time speaking with you about your symptoms and what your goals are for this treatment.

DAY 1

Adjustment + Therapy

After the exam, based on Dr. Shelley's findings, you will receive manual manipulation and/or muscle therapy + e-stimulation to begin to relieve your symptoms and pain. She will also discuss pain management.

DAY 2

Report of Findings

The same day of your initial appointment, we will schedule a second appointment to sit one-on-one with Dr. Shelley and discuss your treatment plan to get your body healed and pain free.

What is the cost?

We have many affordable options for our patients to alleviate any hefty bills. During your second appointment, the 'Report of Findings', Dr. Shelley will go over your treatment plan with you, what your insurance covers, and what the cost will be for you. We have pay-in-full, pay-per-visit, and monthly payment plans for our patients who do not have insurance or whose insurance does not cover Chiropractic services.  We accept CareCredit as another financial option.

 

We are "in-network" with most major insurance carriers and our Wellness Director deals with your insurance company so you don't have to. Most insurance plans cover therapy including deep tissue manual therapy (think therapeutic massage), and is included in your treatment plan.  If there is a cost, we will let you know.

At Coughlin Chiropractic, we respect, honor, and value our active and retired military personnel. While Tri-Care does not allow us to be "in-network" we offer anyone with an ACTIVE or RETIRED current military ID a 10% discount on treatment plans.

X-Rays + Medical Equipment

 Yes we do X-Rays based on Dr. Shelley's recommendations and those are part of your new patient exam. We also carry what is called "durable medical equipment" which is items like a lumbar brace, cervical traction collar, orthotics, or TENs unit. Our Wellness Director confirms with your insurance if these items are covered; if not, they can become part of your payment plan. We also carry organic, high grade supplements to help rebuild your body's natural defense system. Because our office believes in wellness and the body's natural ability to heal itself, we have ensured we offer our patients products that align with our belief.
